JourneyCare: Business Intelligence for Better Patient Care

As the largest nonprofit provider of palliative and supportive care in Illinois, JourneyCare was looking to introduce new reporting tools and analytics dashboards that would inform better patient care decisions and improve revenue cycle operations. By working with Prime TSR to define requirements, create an operational readiness roadmap, build the data warehouse, and launch a self-service delivery model, JourneyCare created the foundation for reporting and operational excellence that reflects its history of innovation.

One Common Mission

Serving nearly 3,000 adult and pediatric patients and their families each day, JourneyCare is comprised of three mission-driven organizations: Horizon Hospice and Palliative Care, JourneyCare, and Midwest Palliative & Hospice Care Center. Together, the three agencies offer more than 100 years of experience caring for patients and families facing serious illness.

Under the common mission of transforming lives and building a community where care is available to all, JourneyCare’s leaders also had the common goal of bridging their IT and enriching patient data to help improve decision making.   

With an impact on patient lives and exposure to sensitive information, Prime TSR approached the project with the same empathy and urgency as JourneyCare. What came next, was a four-phase business intelligence roadmap consisting of operational readiness, data warehouse readiness, data warehouse implementation, and a BI self-service solution.

BI Executive Advisory and Operational Excellence

Together with JourneyCare’s executive team, we created a strategy and execution plan to move the organization toward BI operational excellence. As a part of our approach, we led a strategic technology assessment, recruited and on-boarded several executive leaders, and outlined a new plan to upgrade JourneyCare’s current infrastructure that correlates with their growth goals. 

Next, we led an effort to review potential analytics platforms while matching their internal KPIs and the capabilities of their vendors with the current needs of their business. We identified which systems are core to their success in addition to our recommended approach.

Confident, Clear Decisions Using Enhanced Reports

With millions of sensitive patient records flowing through several mission-critical EMR systems, JourneyCare was looking for a better way to use data to drive executive decisions with confidence and clarity. As part of our strategy, we identified KPI alignments and reporting between executives, staff, and clinical teams as the top priority.  

Finally, we worked together to identify opportunity areas where existing reports and databases could be improved to drive data integrity across the organization. This resulted in 22 types of new reporting dashboards being optimized or created, in addition to enabling their PowerBI reporting capabilities.

Enriching Existing Dashboards and Exposing New KPIs

After the fourth phase of our involvement, JourneyCare was able to cross-reference basic, yet critical patient data such as referral sources, referrals by diagnosis, the average length of a patient stay, and much more. 

In addition to new data sources that enriched existing dashboards, the self-service reporting feature allowed users to build reports that expose new KPIs. For example, JourneyCare was able to improve its clinical resource allocation for better patient care while improving revenue cycle operations by having more data-driven clarity on the money they should be collecting.
